What to Expect:
You’ll meet with a Care Manager who understands the system and is here to help you navigate it. We’ll check in regularly, plan ahead with you, and jump in fast when problems pop up.
This service is free for Medi-Cal members and available $60/hour for others. Reach out to us to see if we’re a fit for you.
Alex James
Alex (she/her) serves as the Care Manager at HH & Co. She has more than six years of experience providing case management and has worked with many different demographics throughout Santa Cruz County. Alex brings a person-centered approach to case management and believes that empowerment and collaboration in care leads to better outcomes and quality of life for people. She received her bachelor’s degree from California State University of Monterey Bay in Collaborative Health and Human Services.
Blanca Carrillo
Blanca (she/her) is a Care Manager on the Complex Care Team at HH & Co., bringing 25 years of local nonprofit experience along with bilingual and bicultural skills to her work. She provides Enhanced Care Management to clients with complex needs, including serious mental illness, substance use, and co-occurring medical conditions, building strong relationships that help clients navigate care and stay engaged in their goals.
